TOBACCO:  I will not use any form of tobacco (smokeless, vape or cigarettes, etc.) during Mission: Sylvester.

LANGUAGE/TOUCH:  During Mission: Sylvester I will refrain from any umpleasant language. I will also not use derogatory remarks toward students or other adults

While on the jobside all participants must wear at minumum, a short sleeve shirt, blouse or tee shirt (No tank tops), long pants and som form of closed toed shoes.  The dress code reflects safety concerns for all individuals who are participating in Mission Sylvester. 

Mission Sylvester: We at First Baptist Church, Sylvester, GA want to first and foremost glorify our Lord and Savior Jesus Christ.  We also hold fast to the belief that, we as followers, are able to magnify Jesus in three specific ways:  By being a willing volunteer who will assist in specific community projects for individuals in our local community.  By ministering to these individulas who may or may not know Jesus as their personal savior.  By recognizing that these selected projects directly and personally impact not on the recipient of the project but also impact the individuals who provide the service. 

It is understood that he/she will be supervixed during Mission: Sylvester, and that normal precautions will be taken in the interest of safety and well being.  We agree that FBC Sylvester, Sylvester Georgia, and Mission: Sylvester personnel will not be held responsible for any accident, illness or injury that may occur in connection with Mission: Sylvester. 

I have read fully the Mission statement including the rules, regulating expected dress code, the use of tobacco, the use of inappropriate language, the cooperation with team leaders and fellow participants.  I also understnad and agree that if a release form is not on file and/or signed by a parent or guardian I will not be permitted to participate in Mission Sylvester until said form is completed and signed by a parent or guardian.  

PHOTO RELEASE:  This document serves as a release to appear in phtotgraphs, video tapes, and church website while participating in activities with FBC Sylvester for the purposes of publicity or promotion.
